Application Checklist:

• Eden Grove Academy Student Enrollment Form
• EdChoice Scholarship Form (if applicable)
• Medical statement completed and signed by physician
• Report of Dental Examination completed and signed by dentist
• Request for Student Records completed and signed by parent or guardian (for transferring students only)
• A copy of the student's most recent report card
• A copy of the student's birth certificate
• Custody Papers (if applicable)
• The following additional items are required if applying for an EdChoice Scholarship
  • Proof of residency (copy of recent utility bill)
  • Student's Social Security number
